Living Well with Dementia Services
People with a diagnosis of dementia can access the Living Well with Dementia services, a referral from the memory clinic, your GP or primary care team member can be sent to our postal address. The referral forms are available below.
Memory Technology Library Services
People with dementia, or people with memory problems that may not have a diagnosis of dementia, as well as families and carers can access the services of the Memory Technology Library. You can self-refer through contacting the occupational therapist or a referral can be sent in by you healthcare team.

Healthcare professionals are welcome to make an appointment to get a tour of the service and learn more about what we offer so you can appropriately refer clients to our services.
