Dementia Support Worker

An important aspect of our service is supporting people to remain as independent as they can at home for as long as possible. To help achieve this, our service has a number of dementia support workers who have been trained to support people living with dementia. Their interventions are primarily activity based, and may involve focusing on achieving individual goals, connecting with existing supports and/or supporting the person living with dementia’s participation within their own community.

Typical activities in the home include supporting the client to continue their engagement with activities such as crafts, baking, gardening, music or other identified interests. Support to maintain community connection might include such as going for a walk or a drive and tea/coffee, support to attend social groups, visiting the local library or swimming pool etc. The service provided is based on the client and family’s individual support needs and assessment by the dementia nurse specialist.
